Sub : Refund of Security Deposit

With reference to above, I would like to mention that I have applied for disconnection of Land Line connection (Telephone No. --------) on 21.08.14 and subsequently returned the telephone instrument etc. to your office on 23.08.14 along with Application Form duly filled in, for refund of security deposit. The copy of the said form is attached herewith.

As the receipt of security deposit in original is not traceable and therefore, the Indemnity Bond on Stamp Paper of Rs.10/- duly signed and notarized dtd.25.08.14 along with Affidavit are attached herewith with a request for processing refund of the security deposit at an early date.

Meanwhile, I would like to state that the recent Telephone Bill dtd.05.08.14 for Rs. -----/- have been paid on 24.08.14 and the xerox copy of the receipted bill of Telephone No.---------- is attached herewith for your ready reference.

With above submission, I would request you to please arrange to refund the security deposit as early as possible.
