iii)             Xerox copy  of your Receipt -----------------

I received your above cheque being excess payment of premium on my above Mediclaim Policy which I deposited in the Bank in time.

Unfortunately after a long time, only on  19.02.14, . the Bank returned the cheque to me . As the cheque need revalidation, I enclose the original cheque for revalidation alongwith a Xerox copy of returning Memo 

Kindly revalidate the cheque and return the same to me for re-depositing in the Bank .
