As I am shifting from my existing place of stay ( 21/1, Chandani Chowk, New Delhi – 101 212)  to  12, Dariya Ganj, Flat no 303, 3^rd floor, New Delhi 101201, I request you to record my new address in your book and transfer the LPG connection  to the said address which is within your LPG  distribution area.

I enclose herewith Xerox copy of my PAN Card as a proof of my identity and a Xerox

copy of New Delhi Corporation current Tax payment receipt and a Electricity bill  as

address proof and I trust it is sufficient for effecting the transfer  of  my LPG

connection to my new address.
