We hereby certify that White Paper material was transported by our Truck No-------- ---- as per Consignment Note No.----------- dtd.25.09.13 from Polymar Works, New Delhi to Rocket Sales Ltd., Khapoli, Navi Mumbai Works at the owner’s risk. Unfortunately, the goods were found to be in damaged condition on reaching its destination due the factors which were totally beyond the control and power of the driver of the said vehicle in transit and because of jerks and bolts and unfortunate event, the following damages/shortages were caused to be consignment as detailed herein below:

This certificate is strictly issued without prejudice to our right and without admitting/undertaking of any liability on account of the damage/shortage caused due to said unfortunate event on the basis of the survey report. This certificate is being issued at the specific request of the consignor that they need the said certificate for the purpose of lodging a claim with its insurance company as the said consignment was insured.
