Dear Mr Anil Singh

Please refer to our last week telephonic conversation  - wherein you had assured that our necessary PENDING C FORM FOR THE YEAR 11-12 will be duly informed and handed over to us.

Unfortunately, till date (already a week has passed) - we had not heard anything from your end.

And, we would like to  inform you that, the sales tax assessment cases for the year 2011-12 HAS ALREADY BEEN PENALISED TO US FOR RS 2,50,000.00, for your non-submission of your DUE C FORM.  and re-hearing for last &  final date of hearing has been fixed for 16th June  2013.

Please note, as we have already been penalised, We are raising a debit note for Rs 2,50,000.00 on you. 

You are again requested to submit your Pending C Forms within 7 days, so that the same can be presented in the final hearing on 16^th June 2013.

Failing to this you have make a payment of Rs 2,50,000.00 against the attached debit note otherwise necessary legal action will be taken against you.

MATTER IS VERY URGENT & IMPORTANT, and still submission of your forms will save the situation

Please immediately arrange to handover the due forms and oblige.
