AFFIDAVIT (For Marriage/Divorce)

I, Rita Joshi, S/o Late Krishna Joshi residing at Flat No.45/5, 8^nd Floor, 95 Lil Kila  Street, Post : Khapoli, Dist. Navi Mumbai, Maharashtra aged about 51 years by faith Hindu by occupation Service, do hereby solemnly affirm and declare as follows :

1.    I Mrs Nilam Joshi, aged about 23 years by faith Hindu by occupation Teacher.

2.    That After Marriage with Mr Snajay Sinha, I am also known and by the name  “Nilam Sinha”.

3.    That the names “Nilam Joshi” and “Nilam Sinha” are the names of one and the same person i.e. my daughter Nilam Joshi.

4.    All the documents including bank account/fixed deposit/any other legal documents which were in the name of Nilam Joshi will be henceforth changed as “Nilam Sinha”.

That the above declaration made by me is true to the best of our knowledge and belief.

I, Raju Joshi, S/o Late Krishna Joshi residing at Flat No.45/5, 8^nd Floor, 95 Lil Kila  Street, Post : Khapoli, Dist. Navi Mumbai, Maharashtra aged about 51 years by faith Hindu by occupation Service, do hereby solemnly affirm and declare as follows :

1)  That my daughter Miss Nilam Joshi, aged about 15 years by faith Hindu by occupation Student.

2)  That my above daughter is also known and by the name “Nilam Sinha”.

3)  That the names “Nilam Joshi” and “Nilam Sinha” are the names of one and the same person i.e. my daughter Nilam Joshi.

4)  All the documents including bank account/fixed deposit/any other legal documents which were in the name of Nilam Joshi will be henceforth changed as “Nilam Sinha”.

That the above declaration made by me is true to the best of our knowledge and belief.
